Case Management

The case management component is designed to provide intensive social support to participants who are working to overcome multiple barriers to obtaining success in a long-term career path. These activities may be performed by a professional social worker in contract tot he Bridge Class, career or job coaches at employers or social service agencies and long-term through volunteer mentors within the company or in the community.

Each participant completes a comprehensive psychosocial assessment, designed to capture any relevant information regarding their life and barriers to success. After assessment, Bridge participants are assisted with goal formulation and case planning. This process deals with both short-term goals (six months) and long-term goals (five years). While in training participants are monitored for participation and attendance. When someone is abbsent, the case manager will attempt to contact this person and deal with any barriers that might have come about. Likewise, the case manager is available to address any issues that might come about as a result of the group process in the class setting. Many times participants show a lack of interest or non-participation that might be caused by personal problems going on in their lives. In these cases the case manager is there to provide needed support in dealing with these issues.
